Sustainability is the process of hard-wiring or locking in the structure, process, or outcome improvements that were made during the QI implementation phase. Sustainability is how you work to ensure that your work is continued on after you.
A variety of features may contribute to sustainable change in an organization dependent upon the objectives of the DNP project. The following are some examples:
- the engagement of senior leaders in the administration early in the planning of the project
- the inclusion of mentors or champions from the organization to facilitate the change and continue processes
- the integration of documentation of the practice change into the Electronic Health Record
- the availability of financial resources for maintaining the process improvement beyond the project end
- a clear need for the practice change to occur within the organization
- insertion of newly developed processes as part of new staff orientation and/or organizational policies
- establishment of an ongoing sustainability measurement plan for collection of outcomes and quality improvement activity data (e.g., structure, process, and outcome measures, frequency of chart audits, ongoing use of benchmarks and target goals)
Refer to your implementation theory when you develop your sustainability plan for your DNP Project. The sustainability plan needs to be specific to your project. The research that forms the foundation for your DNP project will give you insights into the specific elements to consider when your write an actual sustainability plan for your initiative:
- Review Ch 8 in Ogrinc et al and the attached AHRQ pptx as resources
- Analyze the leadership role as it relates to the project - assess group performance at the site and the degree to which goals are met.
- What process does the site use to evaluate leader and initiative effectiveness? Can you use this structure to demonstrate your leadership success?
- What is the succession plan within your site for your leadership role? Who are you investing time into mentor to serve as the lead going forward?
Sustainability Suggestions
- Sustainability and spread - establish a formal plan with the site (if not already) including measures. Review Ch 8 in Ogrinc et al and the attached AHRQ pptx
- Analyze the leadership role as it relates to the project - assess group performance at the site and the degree to which goals are met.
- What process does the site use to evaluate leader and initiative effectiveness?
- If not formalized, revisit strategies from 814/817.
- Complete the Collaborative Leadership Self-Assessment previously completed in NDNP 817 for comparison - what gains have you made? What domains and behaviors were especially relevant and impactful to your project leadership? Share the results with your CSR and Project Faculty for interpretation.
- Facilitators and barriers of the project - what is the succession plan within your site for your leadership role - are your investing time in mentoring a team champion to serve as the lead going forward? How will the data continue to be collected and assessed?
