In this course, you have learned to do a community assessment to identify and prioritize community needs. Those activities are the first steps of program planning and evaluation. Program planning is an organized approach aimed at meeting the needs of individuals, families, groups, or communities. The process includes planning, implementing and monitoring the program, as well as evaluating health outcomes. Interventions are directed at initiating change that will improve the community’s health and prevent disease. This module will focus on the steps (after assessment) in the program planning and evaluation process.
