Overview
Health care reform and the subsequent new health care delivery models such as the Patient Centered Medical Care Home (PCMH) and the Accountable Care Organization (ACO) have created high expectations for increasing access to care, improving quality, reducing medical errors and containing cost. RN care coordination is integral to achieving these goals. Care Coordination is an evolving field with a growing body of evidence.
Objectives
At the conclusion of this module, the learner will be able to:
- Define care coordination, case management and transitions of care.
- Describe the purpose of case management.
- Describe the role of the RN in care coordination.
- List two community case management settings.
- Discuss ethical issues related to case management, screening and referral.
